The Metabolism of Potassium in Diabetic Acidosis *

چکیده

Although it has been known for some time that a loss of potassium from the body occurs in fasting3' 18 and certain types of dehydration,5' 13 and recently that the replacement of this potassium during parenteral therapy of the dehydration due to infantile diarrhea may materially reduce the mortality rate,9 many of the changes that occur in the metabolism of potassium during the state of extreme dehydration associated with diabetic acidosis and coma have remained obscure. Potassium is lost from the body during the development of diabetic acidosis,1' 6 and low serum levels of potassium have been reported during recovery.7 19 ,2325,28 Occasionally there have been associated symptoms attributed to these low levels.23 28 A study of potassium metabolism in diabetic acidosis was undertaken in the hope of defining more clearly the changes in potassium which occur during recovery from severe diabetic acidosis and, if possible, to ascertain whether it is necessary to replace potassium in these patients.

Acid-base disorder analysis during diabetic ketoacidosis using the Stewart approach--a case report.

This case report presents a 49 year-old female with type 1 diabetes admitted to the intensive care unit with acute respiratory failure and severe diabetic ketoacidosis with an initial measurement of blood glucose level of 1,200 mg L⁻¹, pH 6.78, serum HCO₃ ⁻ 3.2 mmoL L⁻¹ and BE -31.2 mmoL L⁻¹. Analysis of the blood gasometric parameters with the Stewart approach and the traditional Henderson-Has...

Effect of acute metabolic acidosis and alkalosis on acetate and citrate metabolism in the rat.

An enhanced urinary excretion of citrate after administration of alkali and a diminished citrate excretion after administration of ammonium chloride were first observed by Ostberg in 1931 (1). More recently, it has been pointed out that the quantity of citrate excreted in the urine is not a direct consequence of acidification or alkalinization of the urine as believed by Ostberg, but rather is ...

Diabetic Ketoacidosis and Hyperosmolar Nonketotic Diabetes Mellitus

Diabetic ketoacidosis (DKA) is the culmination of diabetes mellitus that results in unrestrained ketone body formation in the liver, metabolic acidosis, severe dehydration, shock and possibly death. Hepatic lipid metabolism becomes deranged with insulin deficiency and nonesterified fatty acids are converted to acetyl-co-enzyme A (acetyl-CoA) rather than being incorporated into triglycerides. Ac...

Distal renal tubular acidosis with severe hypokalaemia, probably caused by colonic H(+)-K(+)-ATPase deficiency.

We describe a 21 month old male infant who presented with failure to thrive associated with severe hypokalaemia and metabolic acidosis, together with hypomagnesaemia. Evaluation revealed marked renal and probable faecal potassium wasting, distal renal tubular acidosis, mild urinary magnesium wasting, and a normal gastric pH (gastric H(+)-K(+)-ATPase). Hypokalaemic forms of metabolic acidosis, s...
